Stabilize API tracing and stack symbols

Replace ApiTrace callback-side formatting with a compact synchronous monitor ioctl path to avoid stack exhaustion on Cygwin and other alternate stacks while preserving trace ordering. Also improve stack symbol resolution by refreshing DbgHelp's module list on misses and wiring symbol-change notifications so StackView updates entries as names become available.
