feat(reviews): adversarial review + lift-committee tooling

build-packet, run-holistic, run-perskill, lift-committee (2-round
propose+cross-examine), aggregate; plus raw JSON from gpt-5.5-pro /
gemini-3.1-pro / deepseek-v4-pro reviews and the committee.
