doc: replace tracker ticket links with inline explanations

User-facing docs cited tracker.ceph.com issues as the only source
for known limitations and workarounds. Each citation is replaced
with prose extracted from the ticket: the limitation, its cause, or
the workaround, written to stand on its own.
