ci: use conventional commit message and title for version PR

changesets/action v2 renamed v1's `commit`/`title` inputs to
`commit-message`/`pr-title`. Set both so the release commit and PR
read `chore(release): version packages` instead of the default
`Version Packages`, matching the rest of the history.
