fix(ui): improve Team setup recovery (#1561)

## Description

Improve Team setup recovery and Project repair UX: keep stale states fail-closed, route actionable repair work to Projects, preserve useful labels, remove transient layout shifts, and content-hash the staged Viewer bundle URL.
