Restructure plan.md into autoresearch-style autonomous experiment loop

Replaces the manual 7-step improvement process with a persistent,
metric-driven loop that an agent can follow autonomously. Adds append-only
score history, ideas backlog, what's-been-tried with outcomes, dead ends
for anti-thrashing, and concrete anti-thrashing rules.
