F1=56.3%: Fix read crash, budget investigation, missed patterns

- Fix _execute_read ValueError crash on malformed line ranges
- Add BUDGET YOUR INVESTIGATION instruction (stop chasing dead ends)
- Add FREQUENTLY MISSED PATTERNS checklist (metric tags, lock scope, etc)
- Reduce max_tool_rounds from 40 to 30
- Run all PRs in parallel (no 10-PR cap)
- Improve evaluator judge prompt for semantic matching
- Add asymmetric cache trust, empty ORM updates to system prompt
