Revert to freeform+extraction with baseline prompts + reliability fixes

report_issue tool architecture regressed F1 (51% vs 55.4% baseline).
Restores freeform text review + structured extraction, baseline system
prompt with specific bug examples, and confirm-before-reporting review
prompt. Adds retry logic for 429 rate limits and reduces parallelism
to 5 to stay under token rate limits.
