fix(P0): assistant-terminal guard fired on OpenCode's normal mid-turn continuation shape — every tool-using turn died at its first continuation pass. Revert both throw sites to leave-untouched (the provider serializer owns wire termination); keep the blank-shell re-anchor arm.
