fix: CodexDriver tracking in code-0 auto-restart + dead slot skip

- autoRestartIfIncomplete now tracks respawned CodexDrivers in
  activeCodexDrivers (was only done in crash respawn path, not code-0)
- Dead slot auto-cleanup skips driver-managed Codex slots
