device: version region slot ownership

Encode the active generation in slot state and publish mutable
metadata atomically. Lookups can then reject concurrent slot reuse
without copying region identity into per-block shared memory.

Extend lifecycle stress with non-owner slot probing while preserving
the table-based submission path and its issue-rate profile.
