regions: carry device context without shared memory

Resolve active device region identity after immediate-path routing and
carry the exact issuer, generation, and hints through proxy, IBGDA, and
GDAKI submission.

This removes the persistent block shared-memory cache, preserves the
existing transfer device symbols, and keeps regions functional when an
application provides no shared memory.
