Simplify session lock recovery

Extract persistent-session locking to a new helper module, remove automatic stale-lock stealing, and return stale-lock errors with manual cleanup guidance. Add lock tests and refresh docs/rules to explain the new stale-lock behavior; also keep final agent exit codes for timed-out children consistent with forced termination.
