feat(dx): instructions, PQL resource, README sync check, release prep (Stage 4) (#13)

- instructions: rewrite SERVER_INSTRUCTIONS - orientation (get_instance_info/
  list_toolsets), name->UUID resolution, pagination + sparse fields, PQL
  pointers, epics flow.
- pql: expose the full PQL reference as the resource://pql-reference MCP
  resource (markdown) alongside the get_pql_reference tool.
- docs: scripts/check_tool_docs.py enforces README tool tables match the
  registry (checker, not generator - tables keep curated descriptions);
  wired into CI. Documented the 39 missing tools; all 142 now covered.
- release: add CHANGELOG.md, bump version to 1.1.0, replace the upstream
  Docker Hub workflow (makeplane org) with a ghcr.io publish workflow
  (v* tags, GITHUB_TOKEN, amd64+arm64).

129 tests, 57% coverage.
