Add default-off DSV4 b3 hybrid decode path

Keep MLX gather_qmm for 3-bit gate projections while fusing the 2-bit up projection with limited SwiGLU and route scoring. Reuse the validated fused down kernel and retain the stock path unless explicitly enabled.
