perf(dsv4): fuse mixed 3-bit gate decode

Cover the six affine DSV4 routed-MoE layers whose gate projection uses
3-bit gs64 weights while retaining the existing 2-bit up and down layout.
Self-test each installed gate-bit layout and expose per-layout coverage.
