chore(cache): log per-512-token segment fingerprints under VMLX_CACHE_HASH_DEBUG

head/tail cannot answer the only question worth asking when a chain matches
fewer blocks than were stored: WHERE do the two prompts stop agreeing.
Comparing two SEGMENTS lines shows the first 512-token window that differs
without dumping thousands of ids into the log.
