feat(m4): rework six high-mechanism cohort skills from pre-registered specs

- five-whys-plus: narrowed to post-localization root-cause; added evidence chain, counterfactual cause test, and explicit stop condition
- kepner-tregoe: focused on Problem Analysis / IS-IS NOT for debugging; de-emphasized DA/PPA sprawl
- red-team: narrowed to security/code vulnerability detection; required reproducible attack path; penalized speculative claims
- map-territory: narrowed to docs/tests/mental-model vs runtime/code reality; agent-executable verification checklist
- occams-razor: added trigger-shrink gate; test fewest-assumption hypotheses first
- triz: narrowed to architecture/API contradictions; separation moves as primary procedure

Each reworked SKILL.md matches all 6 template sections (Overview, When to Use, When NOT to Use, Procedure, Output Contract, Anti-Patterns). All descriptions situation-named ('Use when ...') and <200 chars. Pre-registered rework specs committed under analysis/rework-specs/.
