feat(m4): add trigger cards to 17 remaining contract-identified skills for full trigger-vs-full coverage

Derive complete trigger-candidate set from evals/contracts/ (planned_comparison=trigger)
— 22 skills total, up from the 9 quarantine candidates that already had trigger cards.
Add concise 2-4 step actionable ## Trigger Card sections to all 17 missing skills:
bounded-rationality, effectuation, first-principles, jobs-to-be-done, kepner-tregoe,
leverage-points, lindy-effect, margin-of-safety, occams-razor (adapt trigger-shrink
fast-path), ooda, opportunity-cost, pre-mortem, reversibility, steel-manning,
thought-experiment, triz, via-negativa.

Catalog invariants preserved: 39 skills, no deletions, no scientific-method-v2,
all non-router skills keep When NOT to Use, all descriptions <=200 chars.
No description changes; routing-cases.jsonl unchanged and valid.
