regions: resolve proxy state on submitting thread

Resolve proxy region identity on the request-writing thread while
direct transports make the same context available to every
participating lane.

Build the common proxy request before consuming the active region
predicate so request construction overlaps the global-memory load.
Exact lookup remains confined to the active-region publication path.
