Update PR template and verify its completion in CI (#14069)

Taking inspiration from Beeware, update the PR template and include a
checkbox to confirm the author has read the AI policy. If they have used
AI tools, they are expected to disclose such usage via an Assisted-By
trailer that is also checked via CI.
