chore(github): add issue templates, PR template, and code of conduct

Fills out GitHub Community Standards. Bug report template requires the
Collect Diagnostics ZIP from the Status window, since sensor bugs are
hard to reproduce without the debug log and sensor tree from the
reporter's machine.
