chore(cache): report media token count and first placeholder position in the debug line

If the SAME image expands to a different number of tokens between two turns
of one conversation, every position after the first placeholder shifts and no
block hash can match again. That looks exactly like a broken cache while the
cache is working correctly on a prompt that genuinely is not the same prompt.
These two numbers separate the two cases in one read.
